Required documents and application process:
Contract: After approval of the course proposal, a contract will be sent to the instructor
Insurance: A certificate of liability insurance listing “The City of Half Moon Bay” as additional insured is required for all classes. Insurance can be purchased through the Recreation Department.
W-9: A W-9 will be sent with the contract for new instructors.
Live Scan: The City requires a Live Scan background check every two years for all instructors.
TB Test: Any instructors who work with kids under 18 will need a TB test. We require a new TB test every two years for instructors.
Mandatory Reporting: Instructors will need to read and fill out the Mandatory Reporting Form sent with your contract.
